SAN ANTONIO -- The state is investigating the death of a man at a senior care center in San Antonio.
The man was found dead over the weekend at Meridian Care Monte Vista, north of Downtown near San Antonio College. News 4 WOAI received a tip that the resident was found out of his wheelchair, at the bottom of some stairs outside the facility. The source said it was around midnight when it was discovered the man was not in his bed, but his body was found about an hour later. We have confirmed the Texas Department of Aging and Disability Services is investigating the death.
We went to the facility to ask about the man's death. The administrator was there but didn't want to talk to us. He did tell us that anytime anything unusual occurs they report it.
The Texas Department of Aging and Disability Services said the following concerning the incident:
"We can't disclose details of our investigation while it's underway. In any investigation, our role is to determine whether a facility violated any relevant state or federal regulations."
We've also learned during a July inspection, the state agency found Meridian Care Monte Vista had a total of 31 state and federal violations, ranging from not keeping medical records private to not making sure residents were well nourished. According to the DADS website, most of those violations have been corrected.
We checked with San Antonio police but were told they could not find any recent 9-1-1 calls from the facility's address. We will follow this investigation and let you know what happens.
